As an RN Case Manager you will get to manage patients who may not be quite ready for hospice and are starting out on Home HealthCare / Palliative Care.
With continuity of care being very important to us, with Interim patients are able to transition thru our services keeping the same clinicians (whenever possible).
We are looking for RN's who have prior hospice experience.
This a fulltime Monday thru Friday position and there is some on call coverage as well.
Must be caring, compassionate, detail oriented and a team player.
